• It is necessary for the school nurse to have on file definite information concerning causes of absence.
• Written excuses signed by a parent or guardian are required.
• Students must bring notes excusing their absences upon their date of return. The note Should include:
  • Name of Pupil
  • Date of Absence
  • Reason
  • Parent Signature
  • Day, Time, Phone #

Unexcused absences are treated as irregular absences from school and are referred for discipline.
